I can answer number 1 for you Lindsay;
It is because in 1990 communism hadn't quite fallen yet and in Eastern Europe there where multiple uneconomic smokestack industries which have since gone to the wall but count in setting emissions targets.
Thus Nations like Romania and the Ukraine are way ahead in the game because they were great polluters and we are behind because we weren't and never have been.
